COPO Case Management Program

At Council of Peoples Organization (COPO), we believe every older adult deserves to live safely, comfortably, and independently in their own home. Our free Case Management Program is here to provide personalized support for homebound seniors (60+) in Brooklyn, ensuring they have access to the care and services they need.

Our dedicated team of case managers take the time to understand each person’s unique needs through in-home assessments and one-on-one conversations. We’re here to help with:

  • Nutritious Home-Delivered Meals – Ensuring access to balanced, healthy meals. Options include: Regular, Chinese, Kosher, Caribbean, Polish, Latin, Halal, and Vegetarian.
  • Home Care Coordination – Assisting eligible clients with access to Medicaid and non-Medicaid home care services.
  • Companionship & Social Support – Providing friendly visits and social engagement to reduce isolation.
  • Help with Benefits & Entitlements – Support with applications for Medicaid, SNAP, and other essential programs.
  • Regular Check-Ins & Care Planning – Ongoing support to meet your evolving needs.

Our team includes experienced, multilingual case managers, supervised by professionals with Master’s degrees in Social Work (MSW). We also offer translation services to ensure that language is never a barrier to care.

Who Can Join?

You may be eligible for COPO’s Case Management Program if you:

✔ Are 60 years or older
✔ Are homebound or in need of case management support
✔ Live in Brooklyn Community Districts 2, 6, or 7

How to Get Started

Getting the help you need is easy:

  • Fill out the intake form or call us at (718) 434-3266 ext. 1014.
  • Our intake coordinator will conduct a pre-screening over the phone to understand your needs and determine eligibility.
  • If eligible, you’ll be connected to a case manager who will schedule an in-home assessment.
  • After your home visit, we’ll create a personalized plan and connect you to services, benefits, and regular support.
